Reason for Archiving:

WDT asked the ARRL, are the WINLINK forms being used?

Thank you for your inquiry about the regular – or not so regular -- use of these particular FSD reporting forms.
No one that I know of has been using their Winlink e-mail addresses to send me any of these reporting forms.
Of the forms that you asked about, the FSD-157 (Public Service Activity Report) is now known as

ARES Form 1: Public Service Activity Report.

The FSD-212 (EC/DEC Report) is now know as

ARES Form 2: Monthly DEC/EC Report.

The actual forms FSD-125 NTS Monthly Report form and the FSD-89 NTS Area & Region Net Reports form are rarely used as a way to report their monthly activities. However, most of all Field Organization leaders use the format and the information as presented in these forms to create their own reporting forms that they send in to their Section Leader (STM/SEC/SM etc) and to Headquarters by e-mail.

These forms can be found and downloaded from the ARRL Web page:

I think it would be just fine to archive all the noted reporting forms onto the Winlink Forms archive area.
